CGS Offering Assistance in Preventive Care 

 


We'd like to share the following informative letter to providers from

Dear Provider,

CGS wants to partner with you by providing information on three programs the Centers for Medicare & Medicaid Services (CMS) has implemented to assist you in preventive care for your patients. These programs can help you continue to provide high quality care and perform the preventive programs that can help keep your patients healthy and well.

To this end, CGS wants to improve utilization rates of the Medicare Preventive services, Medicare Diabetes Prevention Program (MDPP), and Chronic Care Management (CCM) services.

  • Preventive Services: Medicare pays for many preventive services. These services include exams, shots, lab test, screenings, counseling, education. Preventive services can help prevent illness from occurring or they can help determine if a person is at risk for certain conditions so that he or she can take steps to prevent these conditions.

  • Medicare Diabetes Prevention Program (MDPP): Services aimed to help prevent the onset of type 2 diabetes in patients who have an indication of prediabetes. Program includes CDC-approved curriculum of training in dietary changes, increased physical activity, and weight loss strategies.

  • Chronic Care Management (CCM): Critical component of primary care for patients with multiple (two or more) chronic conditions expected to last at least 12 months, or until the death of the patient. CMS established separate payment under billing codes for the additional time and resources.
